Procedures

ENROLLMENT OF UNDERGRADUATE PROGRAMS

  1. Midshipman to log-in at MAAP On-line Enrollment System (OES).
  2. Midshipman to accomplish OES-Profiling.
  3. Midshipman to present verification code to the Office of the Registrar.
  4. Midshipman to accomplish OES Clearance.
  5. Midshipman will secure Enrollment Form from the Office of the Registrar through his/her MAAP OES personal account.

Note: All midshipmen/women are required to submit their school credentials from their previous schools upon enrollment to the Office of the Registrar. Also, the Office of the Registrar will post memorandum in your respective areas regarding the schedule of enrollment.

ENROLLMENT OF GRADUATE PROGRAM

  1. Continuing/new student shall coordinate to the CAMS representative regarding his/her intent to enroll.
  2. Continuing/new student shall access and accomplish the Online CAMS-OAES.
  3. Continuing/new student to receive Enrollment Form once enrolled through your CAMS-OAES Account.

NOTE:

  1. CAMS will:
    • assess the courses preferred by the student for enrollment.
    • approve the enrollment and issue order of payment for tuition and other miscellaneous fees.
    • advise the student to send the proof of payment through email.
    • forward the proof of payment to the Finance Department through CAMS-OAES.
  2. Finance representative will process the payment through CAMS-OAES.
  3. Registrar will access the name of students once approved by CAMS and cleared by the Finance.

ENROLLMENT OF TRAINEES

  1. Trainee to receive email from the Registrar regarding instructions for online enrollment.
  2. Trainee to register, enroll, and submit entry requirements through online.
  3. Trainee to receive an email from the Registrar regarding his/her enrollment status and Order of Payment, if paying trainee.
  4. Trainee to receive a link regarding his/her class schedule from the ASTC / MSC / MLC once enrolled.

NOTE: Only those trainees endorsed by the ASTC / MSC / MLC for enrollment will receive online instructions. Please coordinate to the said Departments to reserve a slot.

APPLICATION FOR COMPLETION OF DEGREE PROGRAM

  • Student from 3-1 Scheme, Bridging Program, or with approved extension of maximum residency may apply anytime once he/she completed his/her seagoing service days and academic requirements.

    Student will;

    1. Accomplish the online Application for Completion of Degree Program and Graduation Ceremony Form. Click here to access the link.
      If the system is down, the student may process the application manually.
    2. Submit documentary requirements.
    3. Wait for the instructions from the Office of the Registrar to be sent thru his/her email he/she provided during his/her application.
  • Student from 2-1-1 scheme may apply during the 2nd Semester of his/her graduating year (4th Year Level).
    1. Accomplish the online Application for Completion of Degree Program and Graduation Ceremony Form. Click here to access the link.
      If the system is down, the student may process the application manually.
    2. Submit documentary requirements.
    3. Wait for the instructions from the Office of the Registrar to be sent thru his/her email he/she provided during his/her application.
  • Graduating student shall coordinate to the Department of Shipboard Training regarding processing of his/her Final Grade Report, Placement Report, and Notarized Certificate of Validation, Assessment and Approval.
  • Graduating student shall coordinate to the Department of Academics regarding removal of INC of his/her COMPRE or in any course, if Incomplete. The Registrar’s Completion Form for the Undergraduate Program will be sent thru email by request.
  • References:
    1. ACAT Resolution No. 001-11 dated July 25, 2011, requirements for graduation and application for graduation honors:
      • Candidates for graduation awards and Latin Honors shall submit documents qualifying them for such awards.
      • Basis for graduation awards and honors shall be the final grade of the official MAAP Grading System.
    2. ACAT Resolution No. 009-2019 dated November 20, 2019, policy for residency and graduation requirements for the undergraduate programs:
      • Student who is enrolled in any four-year program should finish the program within seven (7) school years period from the date of first enrollment during the First Year Level.
      • In exceptionally meritorious cases, extensions of residence beyond the maximum residency period of completion may be approved by the President upon recommendation of the ACAT Chairman in reference to the letter of consideration submitted by the concern student.
      • Any student who fails to complete all requirements of the degree within maximum residency period and any approved extension thereof shall be disqualified from the Roster of MAAP Midshipman/woman.

APPLICATION TO JOIN THE GRADUATION CEREMONY AND LATIN HONORS

  • The graduation ceremony at MAAP is conducted every May or December of the current year. The date of graduation is incorporated at the MAAP Academic Calendar forwarded to CHEDRO3 and to all concerned offices.
  • Students from 2-1-1 scheme, 3-1 scheme or with approved extension of residency period, may opt to apply to join the graduation ceremony. Students who have completed their degree program from January to May of the current year, may opt to join the May graduation ceremony provided that his/her completion date is a week before the declared graduation date. Students will then be advised to fill out the online application to join the graduation ceremony.
  • Likewise, students who have completed their degree program from May to December of the current year, may opt to join the December graduation ceremony provided that his/her completion date is a week before the declared graduation date. Students will then be advised to fill out the online application to join the graduation ceremony.
  • Students will receive an email from the Office of the Registrar regarding his/her application status.
  • Deadline for submission of application for graduation is 30 days prior to the graduation date. (Ref: ACAT Resolution No. 001-11)
  • Deadline for the application for Latin Honors is 30 days prior to the graduation date. (Ref: ACAT Resolution No. 001-11)

REQUEST FOR DOCUMENTS THROUGH REPRESENTATIVE

This procedure applies to documents issued by the Office of the Registrar once available for claiming.

  1. For the ASTC/MSC Certificate of Completion, Certificate of Enrollment, True Copy of Grades, Transfer Credential etc.
    1. Download and fill-up AUTHORIZATION TO REQUEST/RECEIVE DOCUMENTS FORM.
      Note: If the form does not appear in the browser, kindly check your "Downloads" folder.
    2. Submit fully accomplished AUTHORIZATION TO REQUEST/RECEIVE DOCUMENTS FORM to the MAAP-Office of the Registrar, Mariveles Bataan with:

      2.1 photocopy of valid ID with signature from the Representative and,
      2.2 photocopy of valid ID with signature from the Requesting Party (Document Owner).

    3. Receive requested documents.
  2. For the MAAP Transcript of Records and/or Diploma
    1. Download and fill-up AUTHORIZATION TO REQUEST/RECEIVE DOCUMENTS FORM.
      Note: If the form does not appear in the browser, kindly check your "Downloads" folder.
    2. Submit fully accomplished AUTHORIZATION TO REQUEST/RECEIVE DOCUMENTS FORM to the MAAP-Office of the Registrar, Mariveles Bataan with:

      2.1 photocopy of valid ID with signature from the Representative and,
      2.2 photocopy of valid ID with signature from the Requesting Party (Document Owner)
      2.3 original copy of Special Power of Attorney (SPA).

    3. Receive requested documents.

Releasing

TRANSCRIPT OF RECORDS/DIPLOMA

  1. MAAP Graduate will:
    1. Accomplish Online Outgoing Clearance
    2. Pay mailing fee in cash thru: Gcash (graduate will receive an email from the Office of the Registrar for the Gcash account number and corresponding amount)
    3. Send proof of payment to registrar@maap.edu.ph with the email subject format: NAME /PROOF OF PAYMENT/CLASS. (ex. DELA CRUZ, JUAN M. / PROOF OF PAYMENT / CLASS 2020.
    4. Receive LBC Tracking Number from the Office of the Registrar for monitoring. LBC Tracking Number/s is/are posted at the Registrar’s Facebook Account.
    5. Wait for delivery.
    6. Scan Acknowledgement Receipt QR Code once the document has been received. The QR Code is sent together with the document.
     

    REMINDER: Please take note that releasing of Transcript of Records and /or Diploma depends on the following:

    • Availability of CHED Special Order Number from CHEDRO 3
    • Fully accomplished Online Outgoing Clearance
    • Paid mailing fee
    • Schedule of releasing of certificates is every Saturday or any day that suits the availability of the personnel.

  2. For Resigned / Terminated / Separated Midshipmen/women
    1. Secure Outgoing Clearance Form at the Office of the Registrar.
    2. Submit fully accomplished Outgoing Clearance for TOR.
    3. Receive fully signed Transfer Credential and True Copy of Grades.

    Note:Transcript of Records will be issued upon receipt of the lower part of the issued Transfer Credential signed by your present school.

TRUE COPY OF GRADES / CERTIFICATE OF ENROLLMENT

  1. Submit a Request Letter addressed to the Registrar stating the purpose of your request, your program, class, midshipman serial number and scheme in pdf format and with your signature on it.
  2. Attach a copy of your valid ID (MARINA/PRC ID, UMID, Passport, SRB etc.) for verification purposes.
  3. Forward your Request Letter to the Office of the Registrar for Approval through email at registrar@maap.edu.ph

Note:Releasing of Requested documents is within 3 working days upon receipt of the request letter.

REQUEST FOR RE-ISSUANCE / REPLACEMENT OF TOR, DIPLOMA OR CERTIFICATE OF COMPLETION

  1. Fill out the google form for your respective concern:
    • For RE-ISSUANCE / REPLACEMENT OF DIPLOMA / TRANSCRIPT OF RECORDS please click this link.
    • For RE-ISSUANCE OF CERTIFICATE OF COMPLETION please click the link.
  2. Pay re-issuance/replacement fee through bank transfer or in cash at MAAP Satellite Office, Manila.

  3. Upload complete required documents.
  4. Monitor your email for the status of your request.
  5. If the requestor is unable to attend to the transaction himself/herself, he/she must email the following at registrar@maap.edu.ph
    1. Special Power of Attorney (for Diploma and Official Transcript of Records only)
    2. Fully accomplished F-087 AUTHORIZATION TO REQUEST/RECEIVE DOCUMENTS FORM.

    3. Photocopy of requestor’s valid ID with picture and signature and a photocopy of his/her representative’s valid ID with picture and signature.
  6. MAAP Transcript of Records and Diploma will be mailed directly to you once paid and complied with the requirements.
